Observation

On 2/15/23 at your monitoring visit the licensor noted the bathroom outlet is not GFCI and TR. Push in plate is not approved by the department. Permanently installed plates screwed in are approved if you wish to cover with a plate. Construction debris on the side of the house accessible to children and in the licensed space is a hazard to children. There is stacked wood with nails, metal railing for the garage door another other construction debris. Additionally, the air conditioner unit in the licensed outdoor play area is accessible to children. The air conditioner fan has blades that can cause a cut to a child. On 2/15/23 during your monitoring visit the licensor observed a empty plastic bag in the licensed outdoor play area It was noted on 2/15/23, the bifold doors in the licensed space were not childproofed and a pinching hazard. On 2/15/23 licensor observed the bathroom in poor condition and in need of repair. Toilet lid in upright position with inside of toilet accessible to children. You said the toilet won't stop running if closed. Toilet is in need of repair. The laminate flooring on the bathroom floor around the toilet is separating leaving gaps in approximately 2x3 area. Surface is no longer cleanable. Bathroom exhaust fan is not working properly.
